Four Warnings Sat Unread at Full Sutton Before Paul Fitzgerald Killed Richard Huckle in an Attack the Ombudsman Said Could Have Been Stopped

The Monstrous Legacy of Richard Huckle: Britain's Most Depraved Predator

FULL SUTTON, EAST YORKSHIRE — Richard Huckle was killed in his cell at HMP Full Sutton on October 13, 2019. He was 33. He was serving 22 life sentences. The fact that still governs the case is not the folklore that grew up around his name. It is a stack of security reports. In the ten days before he died, staff wrote four separate intelligence reports about the man who would kill him. The reports were not analyzed on time. The actions they recommended were not finished. A wing manager did not interview the prisoner the intelligence was about. A referral to the mental health team was not made. Then one officer was on the landing, and another prisoner had to tell the staff that something was wrong.

Huckle was from Ashford, Kent. At sentencing in 2016 he was 30, and the Old Bailey had in front of it 71 counts of sexual assault against children. He had posed as a student, a photographer, and an English teacher inside impoverished Christian communities in Kuala Lumpur, and he had also offended in Cambodia. The Guardian’s account of the sentencing and the later BBC account of his death put the known age range of victims from six months to 12 years, with investigators estimating the true number of children as high as about 200. He kept a ledger that assigned points. The National Crime Agency has described that book as part of how he turned abuse into a private accounts system. Malaysian commentators called the British life sentences too light. No term of years was going to be received there as an answer.

He was not caught by a neighbor in Kuala Lumpur. He was caught because British police had quietly taken over a hidden child-abuse forum and followed the files. The same investigative thread runs into The AEGIS Alliance’s reporting on the Welcome to Video marketplace. Investigators later said it was highly likely he had offended in Britain as well, before the years in Southeast Asia. The decade from roughly 2006 to 2014 was not a gap in his behavior. It was a gap in every system that was supposed to notice a British man moving through churches, classrooms, and family homes with a camera.

Full Sutton is a high-security prison outside York that holds men the state has decided cannot be kept anywhere softer. Huckle was one of them. Paul Fitzgerald, then 30, lived on the same wing. The Prisons and Probation Ombudsman, Sue McAllister, published the homicide investigation years later, in May 2024, and the BBC’s headline was the finding, not the violence: the murder could have been avoided. Fitzgerald had been diagnosed in 2008 with a personality disorder with psychopathic traits. Before the attack he told an officer he had constant thoughts of murder, rape, cannibalism, and torture. Three days before the killing he told his offender manager he was planning violence. Both officers reported the remarks. There were other incidents, including intelligence about a note offering money to attack a prisoner who was not Huckle. McAllister’s report says each piece was considered in isolation. Nobody added them up.

On October 13, Fitzgerald was alone with Huckle inside Huckle’s cell for 78 minutes. Prosecutors later described a prolonged assault. Paramedics confirmed death at 12:30 p.m. Only after another prisoner raised the alarm did staff remove Fitzgerald and start first aid. A Hull Crown Court jury convicted Fitzgerald of murder on November 23, 2020, after about an hour of deliberation. The next day he was sentenced to life with a minimum term of 34 years. He told the court, in substance, that he wanted Huckle to feel what the children had felt. That motive is not a security plan. A prison does not get to treat a forecast of serious violence as someone else’s moral project.

The ombudsman’s recommendations were procedural because the failure was procedural. The governor was told to make sure security intelligence that signals violence is analyzed promptly, that recommended actions are actually carried out, and that wing managers interview any prisoner whom the intelligence suggests may be about to hurt someone. The Ministry of Justice answered, publicly, that “significant improvements” had been made since the killing “to better spot and tackle those who pose a risk to other prisoners.” That sentence is a claim. The report underneath it is a list of jobs that were not done in October 2019.

Huckle’s crimes remain what the sentencing judge was looking at in 2016: calculated access to children who had less power than he did, a professional costume, a written tally, and a trail of files that only became visible when police were already inside a hidden forum. Communities in Malaysia were left with the betrayal as well as the injuries. The British sentence did not repair any of that, and neither did the manner of his death. What the death exposed was a second institution, a maximum-security prison, failing to read its own warnings about a different dangerous man.

Full Sutton’s own monitors have since described a calmer jail, which is worth putting next to the 2019 file rather than instead of it. The Independent Monitoring Board’s annual report for 2025, published in May 2026, said the prison held 580 men on December 31, 2025, most of them assessed as a serious risk to security or to the public, including a small number of terrorism-related prisoners and others whose crimes are notorious. The Board’s main finding echoed the year before: Full Sutton was, in its view, safe and generally calm, and governors were approaching security and welfare proportionately. Assaults on staff almost halved in 2025 compared with 2024, and the number classed as serious also fell. There were six deaths in custody in 2025, one fewer than in 2024. One of those deaths had been endorsed by the ombudsman as natural causes. A separate ombudsman report, published in November 2025, criticized the same prison for using restraints on a 76-year-old prisoner, David Howarth, that were not proportionate to his health and escape risk. The jail that missed Fitzgerald is not frozen in 2019. It is also not finished being audited.

The useful lesson from Huckle is the unglamorous one. A man who spent years constructing a charitable identity was stopped by investigators who followed files, not by the communities he had fooled. A man who announced, inside a high-security prison, that he was going to be violent was not stopped by the people who wrote the warnings down. Both failures are about information that existed and was not acted on. The ombudsman’s report et les BBC’s account of it are the documents. The prison’s later claim of improvement is a separate document, and it has to be read against the six deaths the monitors counted in 2025, not against a press line.
