SOUTHERN CALIFORNIA — Kaiser Permanente ran a quiet internal experiment in Southern California on the COVID-19 swab tests the country was using in the first half of 2020. Office staff on the business side were split: half actually used the swab, half submitted unused kits. The lab was not told. Unused swabs came back positive. In one California office group, every blank came back positive. That is not a diagnostic. That is noise dressed up as a result.
The trial was not published as a journal paper at the time. Workers and clinicians who saw the numbers described false positives on blanks, and a positivity rate that could not be trusted to decide who got isolated, who got fired, and who got a hospital bed. Emergency-use PCR assays were sensitive enough to pick up dead viral fragments weeks after a person was no longer infectious. Cycle-threshold cutoffs were not standardized. Two labs could call the same sample opposite ways.
Spain ditched Chinese kits that doctors put at about 30 percent accurate. The U.K. said millions of imported kits were unreliable. India called the accuracy poor. A 2021 Journal of Occupational and Environmental Medicine study of SARS-CoV-2 RT-PCR in a large U.S. workforce later found that of 239 positives after pre-employment tests were stripped out, 54 — 22.6 percent — were deemed false positives. That is the same class of problem Kaiser’s people had already watched on unused plastic.
The Tests Got Better. The First Year’s Decisions Did Not Get Rewound
Later FDA-authorized PCR tests got better. The CDC now calls PCR the gold standard. Rapid antigen tests miss people who are actually infectious; specificity sits around 99 percent when used as directed. None of that rewinds 2020. Schools, jobs, travel, and hospital beds were allocated on a diagnostic that a healthcare giant’s own staff had already watched fail a blank swab.
