Garnett and pleaded guilty to interstate stalking resulting in death and cyberstalking resulting in death in the December 4, 2024 killing of UnitedHealthcare chief executive Brian Thompson. There was no bargain. Southern District prosecutors said they will ask for life imprisonment when he is sentenced on December 18, 2026. The guidelines sheet in the same hearing pointed to a range of roughly 24 to 30 years. The judge, not the range, will pick the number.<\/p>\n<p>The plea is the legal fact that swallowed the original headline about letters. In late 2024 and early 2025, Mangione was answering mail from strangers while detained. One of those strangers was a mother published under the name Karen, not his own mother, whose daughter has a rare condition and whose UnitedHealthcare denials, she said, left the girl wrecked after 60 hospital days in a single year. Journalist Ashley Shelby printed that exchange on her Substack, <a href=\"https:\/\/ashelby.substack.com\/p\/warrior-jesus-and-a-letter-from-luigi\" target=\"_blank\" rel=\"noopener\">\u00ab\u00a0Bartleby on Trial.\u00a0\u00bb<\/a> Mangione wrote that Karen&rsquo;s letter was the first to make him tear up inside a cell and asked for a photograph of the daughter for his wall. Karen called the company white-collar criminals and said she would keep fighting the industry in his name.<\/p>\n<p>That correspondence is why a guilty plea did not empty the sidewalk. Thompson is dead. (REUTERS)<\/figcaption><\/figure>\n<h2>What He Admitted, and What the Federal File Already Lost<\/h2>\n<p>Mangione, 28, of Maryland, admitted tracking Thompson to UnitedHealth Group&rsquo;s investor conference and shooting him outside a Midtown hotel. The <a href=\"https:\/\/www.justice.gov\/usao-sdny\/pr\/luigi-mangione-pleads-guilty-connection-his-murder-united-healthcare-ceo-brian\" target=\"_blank\" rel=\"noopener\">Justice Department statement<\/a> says agents recovered a firearm and other evidence after his December 2024 arrest at a McDonald&rsquo;s in Altoona, Pennsylvania. In court he described building a gun with a 3-D printer and fitting a suppressor. Judge Garnett told him he will still have to serve a large share of whatever term she imposes.<\/p>\n<p>The death-penalty count is already gone. On January 30, 2026, Garnett dismissed the murder-through-use-of-a-firearm charge that carried capital exposure. She called her own earlier analysis of the stalking predicate \u00ab\u00a0tortured and strange\u00a0\u00bb and held that it did not qualify as a crime of violence under the statute prosecutors needed. The government did not appeal. Two stalking counts with life maximums were what remained. Those are the counts he has now admitted.<\/p>\n<p>U.S. Attorney Jamie McDonald said there were \u00ab\u00a0no accommodations or concessions\u00a0\u00bb and that prosecutors remain free to seek the maximum. That is how a guidelines range of two to three decades and a courtroom request for life can sit in the same file. Garnett has discretion. December 18 is when she uses it.<\/p>\n<p>The New York state murder trial that had been set for September 8 was postponed. Defense lawyers want the state case thrown out on double-jeopardy grounds after the federal plea. State prosecutors say they will litigate the motion. A state judge had already tossed two terrorism counts in 2025 as legally insufficient, leaving a second-degree murder charge that carries 25 years to life. A defense fund took gifts that included a reported $30,000 anonymous check. Street murals and \u00ab\u00a0deny, defend, depose\u00a0\u00bb slogans turned a homicide into a referendum on prior authorization. That climate does not excuse the shooting. It is the weather in which a CEO was shot on a sidewalk and a federal defendant later found a worldwide inbox.<\/p>\n<p>Karen&rsquo;s letter is useful because it is specific. A rare-disease parent describing denials is not a manifesto. It is the ordinary grinding machine that millions of patients already know. Mangione&rsquo;s reply treated that grinding as moral proof. The courtroom treated the Midtown shooting as a crime. Both documents can be true at once. (Bartleby on Trial\/Substack)<\/figcaption><\/figure>\n<h2>A Sentence Will Not Rewrite an Insurance File<\/h2>\n<p>Federal prosecutors will argue that a targeted killing of a health-insurance executive, planned across state lines and carried out with a homemade suppressed weapon, belongs at the top of the statute. The defense will argue that the remaining counts are stalking theories, that the capital count already failed, and that a guidelines range in the mid-20s is the honest number. Garnett has already shown she will cut theories she finds sloppy. That cuts both ways. It does not make her a soft landing.<\/p>\n<p>Thompson left a family and a company that still processes claims the same way it did the week before the shooting. UnitedHealth Group replaced a CEO. It did not replace the prior-authorization machinery that filled Mangione&rsquo;s inbox with strangers. That mismatch is why the letters keep circulating. They read like proof to people who have fought a denial. They read like grooming of a myth to people who buried Thompson. A federal plea does not settle that argument. It only ends the question of who pulled the trigger.<\/p>\n<p>The state case may die on a motion. If it does, New York still has a dead executive and a federal sentence. If it does not, Mangione could face stacked terms that make the December hearing look small. Double jeopardy in this posture is a fight lawyers love and families hate, because it turns a homicide into a seminar on successive prosecutions.<\/p>\n<p>Readers who follow corporate-target violence through The AEGIS Alliance already know the pattern. A spectacular crime gathers a political choir. The choir does not sit at counsel table. Thompson&rsquo;s relatives will. So will a probation officer with a guidelines worksheet.
