The patient was already asleep. Potter scrubbed out, took the call, and later posted what happened. The clip spread because it put a utilization review on the same timeline as a scalpel.<\/p>\n<p>UnitedHealthcare answered that it had already approved the procedure and the stay, that an office-side error triggered the outreach, and that it would never expect a physician to interrupt care to return a phone call. In early January 2025 she said she had just finished a slate of bilateral DIEP cases and tissue-expander cases when the insurer&rsquo;s call came in during one of the DIEP procedures. The representative wanted a diagnosis and a justification for an inpatient night. The surgery itself, she said, had already been pre-approved.<\/p>\n<p>That combination \u2014 approved procedure, contested bed \u2014 is the everyday grammar of American insurance. Prior authorization is sold as a check against waste. In practice it is a second medical opinion delivered by someone who is not in the room and may not have the file. Potter&rsquo;s decision to step out is her own. Her point was that a system which can reach a surgeon mid-case has already decided that the clock in the operating room is negotiable.<\/p>\n<p>The posts went across TikTok and Instagram. Commenters treated the call as proof that \u00ab\u00a0utilization management\u00a0\u00bb is not a back-office function. It is a voice that can enter a sterile field. Coverage described letters and the threat of a defamation fight over how she characterized the call and the denial. A company that manages tens of millions of lives has a communications shop. Rural exemptions and faster payments to some hospitals are real policy changes.
